1/3. thrombocytopenia associated with minidose heparin therapy.

    thrombocytopenia developed in an elderly woman who was being treated for an impacted fracture of the right femoral neck. The complication was attributed to heparin because the platelet count decreased each time the drug was administered, and when the drug was stopped, the count increased. By the time of death, the patient's platelet count had dropped to 30,000/cu mm. This case is presented to alert physicians to the possibility of thrombocytopenia with the prophylactic use of minidose heparin.

    With the rising public interest in physical fitness, emergency physicians are seeing an increasing number of stress fractures. early diagnosis, followed by conservative management, allowed a long-distance runner with a compression type stress fracture of the femoral neck to return to running. The biomechanical cause of this injury may be related to erosion of the sole of the running shoe that reduced its shock absorption and increased the potential for injury.

    Traumatic hip pain is a commonly encountered complaint in the emergency department. Occasionally, initial radiographs fail to show a fracture. A delayed diagnosis can result in significant patient morbidity. Diagnostic algorithms have been formulated to evaluate the patient with hip pain and negative initial radiographs. The auscultatory percussion technique can alert the physician of the presence or absence of an occult hip fracture. Consequently, the physician may order a more sophisticated imaging technique.
