1/3. Group A streptococcus and streptococcal toxic shock syndrome: a postpartum case report.

    Streptococcal toxic shock syndrome from group A streptococcal disease is rare in the postpartum period, yet it is associated with high morbidity and mortality. early diagnosis and treatment can lessen complications. Midwives can improve provision of postpartum care by being adequately educated about signs and symptoms of this disease, thereby enhancing their ability to recognize it and obtain the appropriate referral or collaborative medical care. A clinical presentation of postpartum streptococcal toxic shock syndrome, including provider follow-up, is presented.

2/3. Fatal toxic shock syndrome associated with clostridium sordellii after medical abortion.

    endometritis and toxic shock syndrome associated with clostridium sordellii have previously been reported after childbirth and, in one case, after medical abortion. We describe four deaths due to endometritis and toxic shock syndrome associated with C. sordellii that occurred within one week after medically induced abortions. Clinical findings included tachycardia, hypotension, edema, hemoconcentration, profound leukocytosis, and absence of fever. These cases indicate the need for physician awareness of this syndrome and for further study of its association with medical abortion.

3/3. Delayed postpartum uterine dehiscence. A case report.

    BACKGROUND: Delayed postpartum uterine dehiscence with hemorrhagic shock may be caused by inadequate treatment of postpartum endomyometritis. CASE: A woman developed a delayed uterine dehiscence six weeks postpartum. Inadequate antibiotic treatment for postpartum endomyometritis was thought to be the etiology. The patient required a hysterectomy for definitive treatment because of associated hemorrhagic shock. CONCLUSION: Only one other case of uterine dehiscence that resulted from endomyometritis has been reported. Broad-spectrum antibiotic therapy is indicated in postpartum endomyometritis to avoid uterine dehiscence with hemorrhagic shock.
