1/3. Malformations of the lungs.

    The postoperative fatal cases following lung resections and those following drainage of pleuropulmonary suppuration have been evaluated. It has been shown that prognosis is significantly better when resections of the lung are carried out than when septic processes in the pleural cavity occur, although the operative interferences in cases of resection are much more extensive than the insertion of a suction catheter for empyema. It was found that with today's methods of anaesthesia and resuscitation even extensive operations on the lung are less risky than bacterial infections.

2/3. Marked elevation in serum alkaline phosphatase activity as a manifestation of systemic infection.

    jaundice of severe bacterial infection is well known. Marked elevation of the alkaline phosphatase has not previously been emphasized as a manifestation of severe bacterial infection. We report 4 patients with bacterial infection manifested chemically by a markedly elevated serum alkaline phosphatase and a minimally elevated serum bilirubin.

3/3. Antigenemia detected by radioimmunoassay in systemic aspergillosis.

    Because of difficulties in antemortem diagnosis of systemic aspergillosis, a radioimmunoassay to an aspergillus fumigatus carbohydrate was developed and evaluated in patients with mycotic or bacterial infections. Antigenemia was detected in sera obtained antemortem from four of seven patients with systemic aspergillosis and in pleural fluid from an Aspergillus empyema but not in control sera or pleural fluid from 43 patients or 27 normal donors. When characterized with reference to onset of disease, antigenemia was an early sign of infection. This study shows the usefulness of the Aspergillus antigen radioimmunoassay for early, specific immunodiagnosis of systemic aspergillosis.
