2/4. Crohn's disease and cheilitis.

    A five-year-old boy presented to his family physician with painless swelling of both lips. One year later he developed abdominal pain, nonbloody diarrhea, weight loss and joint pains. Colonoscopic examination demonstrated patchy erythema, friability and multiple aph-thous ulcers consistent with the appearance of Crohn's colitis, and treatment with prednisone was initiated. Colonic biopsies displayed a chronic inflammatory cell infiltrate, focal cryptitis and fissure formation. The patient's lip swelling relapsed on multiple occasions when steroids were tapered, despite minimal intestinal symptoms of Crohn's disease. The objective of the present report is to alert physicians to this unusual presentation of Crohn's disease and that cheilitis may run a protracted course.

3/4. noma-like gangrenous cheilitis in a child with cyclic neutropenia associated with myeloperoxidase deficiency.

    An unusual case of cyclic neutropenia and hereditary myeloperoxidase deficiency complicated by noma-like gangrenous cheilitis is described. klebsiella pneumoniae and candida albicans were cultured from the involved area. We present this case to increase physician awareness of the possible association of both quantitative and qualitative defects of neutrophils and to stress the importance of the early and effective management of skin infections that can rapidly progress to severe sequelae when associated with profound neutropenia.

4/4. Angular cheilitis: a case for the oral physician?

    It has been suggested that in the future the role of the dental practitioner increasingly will be that of the oral physician. This case report describes the difficulties encountered by a general dental practitioner while investigating and treating a patient with angular cheilitis.
