1/12. Primary mullerian carcinosarcoma of the retroperitoneum: report of a case.

A mullerian carcinosarcoma or malignant mesodermal mixed tumor (MMMT) originated from the retroperitoneum of a 51-year-old woman; only two previous similar cases have been reported. The 1,040 gm tumor was found in the left retroperitoneal space; the center of the tumor was extensively liquified. The uterus, fallopian tubes, and ovaries were unremarkable. The histopathological features of the tumor were typical of MMMT, homologous type. The adenocarcinomatous component of the tumor was immunohistochemically positive for cytokeratin 7 and negative for cytokeratin 20. MMMTs arising in extragenital sites are rare, and most of them arise from the peritoneum. The histogenesis of extragenital MMMTs remains speculative, but the origin from the "secondary mullerian system" is most likely.

2/12. Nasopharyngeal teratocarcinosarcoma.

Congenital germ cell tumors are uncommon. The most common site of origin is in the saccrococygeal region. Teratomas arising from the head and neck comprise a small proportion of this entity, and of these, nasopharyngeal lesions are rare. Also known by various synonyms such as hamartoma and hairy polyp, the teratoma is a well-recognized, and generally benign, clinical and histopathological entity. We present a case of a nasopharyngeal teratocarcinosarcoma associated with a cleft palate and the congenital replacement or absence of the ipsilateral eustachian tube.

3/12. carcinosarcoma and mixed mullerian tumors of the fallopian tube: report of four cases.

We report a 74-year-old woman with malignant mixed mullerian tumor and three women, aged 47, 58, and 76 years, with carcinosarcomas, all primary in the fallopian tube. The tumors grew predominantly intraluminally and were associated with hydrosalpinx. All four patients underwent salpingectomy. Two of them received radiation therapy; one died within 9 months because of the tumor and the second is well 2 years after diagnosis. Of the two women without additional therapy, one is alive with tumor 53 months postoperatively and the other is well 1 year after surgery. Available data suggest that these neoplasms are relatively radioresistant and that their prognosis correlates best with local invasiveness.

4/12. carcinosarcoma of the fallopian tube.

A patient with Stage III fallopian tube carcinosarcoma treated with cisplatin, doxorubicin, and cyclophosphamide, surviving 31 months disease-free after diagnosis, is presented. review of the literature revealed this is the 28th case report of malignant mixed Mullerian tumor of the fallopian tube, and fifth instance in which chemotherapy was used as a primary therapy. Modern chemotherapy appears to have improved survival.

5/12. Carcinosarcomas and mixed Mullerian tumors of the fallopian tube.

Four cases of carcinosarcoma and mixed mullerian tumors of the fallopian tube are presented. Each patient presented with abnormal bleeding and a pelvic mass. All underwent total abdominal hysterectomy, bilateral salpingo-oophorectomy, staging, and cytoreduction. disease was limited to the pelvis in two patients, analogous to FIGO stage IIB ovarian carcinoma; the other two patients had upper abdominal disease, analogous to FIGO stage III. The primary tumors were intraluminal and papillary. There were equal amounts of carcinoma and sarcoma in three tumors; in one, sarcoma constituted only a small intraluminal focus. The sarcoma was predominantly homologous, with foci of heterologous elements present in three tumors. Adjuvant therapy consisted of pelvic radiation in two patients. One patient died of inanition within one year of diagnosis. The other patient, who had the small focus of sarcoma within a stage IIB carcinoma, had an 11-year disease-free interval before retroperitoneal recurrence of carcinoma. Two patients received chemotherapy. A stage IIB patient, after pelvic radiation, received Cytoxan and Adriamycin; she is clinically free of disease after 6 years. A stage III patient lived over 3 years after treatment with multiple agents; she responded to Cytoxan and cis-platinum before suffering a systemic relapse and death.

6/12. Fallopian tube cancer.

Primary carcinoma of the fallopian tube is a rare disease and has traditionally been managed in the same manner as epithelial ovarian cancer. However, unlike ovarian cancer, fallopian tube cancer is not routinely suspected and treatment may be delayed. The clinical presentations of seven cases of fallopian tube cancer emphasize the need for accurate assessment of symptoms to ensure early diagnosis and treatment of this disease.

7/12. carcinosarcoma of the gallbladder: a case report and review of the literature.

A case of pedunculated carcinosarcoma of the gallbladder, presenting with common duct obstruction is reported; radiological, operative and pathological findings are described. The patient was treated with cholecystectomy, common bile duct exploration and postoperative radiotherapy applied through a t-tube. Only 20 cases of carcinosarcoma of the gallbladder have been described. The current state of knowledge regarding the natural history of these tumors are reviewed.

8/12. Primary malignancies of the fallopian tube.

Ten cases of primary adenocarcinoma of the fallopian tube occurring over 23 year period are presented. The mean age of the patients was 61.2 /- 9.2 years (range 48-77). Eight of the patients were postmenopausal. Abnormal vaginal bleeding was the most common symptom and was present in six patients. A pelvic mass was detected in seven cases. In one of the patients abnormal vaginal cytology was leading to correct diagnosis. The problem of the disease is a late diagnosis. Eight of the ten patients with tubal adenocarcinoma represented clinical Stage III or IV. Four of the patients are alive more than ten years after the diagnosis. A rare case of carcinosarcoma of the fallopian tube is presented. Only 26 such cases have been described in the literature to date. In addition a case of tubal choriocarcinoma is reported.

10/12. Heterologous carcinosarcoma of the fallopian tube. A case report.

Carcinosarcomas occur infrequently in the female genital tract. They arise predominantly in the uterus and much less frequently in the vagina, cervix and ovary. They rarely occur in the fallopian tube; only 25 such cases have been reported in the literature to date. The rarity of carcinosarcoma of the fallopian tube prompted this case report.
