1/122. mucoepidermoid tumor of trachea.

    Mucoepidermoid carcinoma of the trachea is rare. Its occurence in a 14-year-old boy is reported here. This case illustrates the typical course of tracheal tumors with clinical manifestations of cough, wheezing, and hemoptysis, the intially reported normal chest roentgenogram, and the common failure to diagnose tracheal tumor for several months. Early use of tomographic studies and bronchoscopic examination in any person with recent onset of airway obstruction unresponsive to bronchodilator therapy is recommended.

2/122. Pseudomesotheliomatous carcinoma involving pleura and peritoneum: A clinicopathologic and immunohistochemical study of three cases.

    Pseudomesotheliomatous carcinoma is a rare variant of peripheral adenocarcinoma of the lung that can manifest clinical, radiologic, and pathologic features similar to malignant mesothelioma. We present three patients with pseudomesotheliomatous carcinoma of the lung. In one patient the carcinoma extended beyond the thorax and extensively involved the peritoneum, mesentery, omentum, and intestines. All patients experienced weight loss and chest pain. All were white men aged 63, 65, and 67 years. Two were smokers and had shortness of breath, cough, and pleural effusion. One had a history of asbestos exposure. No patient developed dyspnea or hemoptysis. One was successfully treated for prostatic carcinoma 18 months earlier. Radiographically, all tumors were pleura-based. Grossly, the tumors spread extensively over pleural (and in one case peritoneal) surfaces and mimicked malignant mesothelioma. Histologically, all tumors were poorly differentiated and necrotic; two tumors exhibited spindle-cell components and desmoplasia. Mucin production was detectable in none, 10%, and 50% of tumor cells. The percentages of tumor cells immunoreactive for Ber-EP4 were 70%, 100%, and 80%; for Leu MI 0%, 90%, and 50%; for epithelial membrane antigen 80%, 80%, and 100%; for B 72.3%, 0%, 90%, and 20%; for polyclonal carcinoembryonic antigen 0%, 10%, and 10%; and for monoclonal 5%, 0%, and 0%. Of these, Ber-EP4 and B 72.3 rendered the most reliable diagnostic results. The clinical, radiologic, and gross and routine histologic findings were similar to those of a malignant mesothelioma; the final diagnosis could be made based mainly on immunocytochemical results. We have reviewed the English and German literature regarding 65 such tumors and present our experience with three additional cases. We emphasize the application of immunocytochemical studies on pleura-based poorly or undifferentiated malignant tumors of unknown origin.

3/122. Anesthetic management of high-risk cardiac patients undergoing noncardiac surgery under the support of intraaortic balloon pump.

    patients with severely impaired left ventricular function, an uncorrectable coronary artery disease, and a recent myocardial infarction are at high risk of cardiac complications after major noncardiac surgery. We present two patients with extensive three-vessel coronary artery disease who underwent intraperitoneal surgery under the support of intraaortic balloon pump (IABP). In one patient, the IABP was inserted urgently because of the development of chest pain with significant ST depression on arrival in the operating room, and the other patient was managed with prophylactic IABP. There were no intraoperative or postoperative cardiac events in either patient. Thus, IABP should be considered in the perioperative management of patients with severe cardiac diseases.

4/122. Pulmonary lymphangitic sarcomatosis from cutaneous angiosarcoma: an unusual presentation of diffuse interstitial lung disease.

    Pulmonary lymphangitic sarcomatosis (PLS) is not much recognized clinically although it shows similar pathological patterns and diagnostic features to pulmonary lymphangitic carcinomatosis (PLC). We report a case with hand angiosarcoma whose chest X-ray findings revealed a diffuse interstitial pattern consistent with lymphangitic spreading. The final diagnosis was made by open lung biopsy. The clinical, diagnostic and pathological features of this disease process are reviewed.

5/122. Advanced insular thyroid carcinoma in a fourteen-year-old girl: twenty-four years of follow-up.

    Insular thyroid carcinoma has become a separate entity among thyroid malignancies. It is regarded as intermediate in aggressiveness between well-differentiated and anaplastic thyroid carcinomas. Reports on the clinical course of children with insular thyroid carcinoma are rare. We report the case of a 14-year-old girl who was admitted to our thyroid outpatient ward in 1975 with a scintigraphic cold thyroid nodule and multiple enlarged cervical lymph nodes. Chest radiography showed metastases in both lungs. After total thyroidectomy and, central and modified unilateral radical neck dissection, the girl was given a dose of 80 mCi 131I. Posttherapeutic scan demonstrated diffuse tracer uptake in both lungs. A second dose of 200 mCi 131I was administered 4 months later and another dose of 150 mCi 131I in July 1976. Subsequently, whole-body scans showed inconspicuous tracer distribution and chest x-rays were normal. The patient was treated with levothyroxine and followed until 1982, when she left Vienna. In 1999, the patient was contacted for reexamination. She has been well and had had two healthy children. The patient was taking 150 microg levothyroxine daily; she had a normal TSH value and her thyroglubulin was 0.3 ng/mL. Chest radiography and sonography of the neck showed no pathological findings. The paraffin sections of the patient's tumor were reexamined in 1999 and demonstrated the histologic characteristics of a poorly differentiated insular thyroid carcinoma (pT4a, pN1a, M1). These findings demonstrate that even in advanced stages, insular thyroid carcinoma treatment can be successful.

6/122. Usefulness of synaptophysin immunohistochemistry in an adult case of choroid plexus carcinoma.

    choroid plexus carcinoma in an adult case is a very rare tumor and difficult to differentiate from metastatic tumors. The authors report a case of a 49-year-old female with choroid plexus carcinoma who previously had multiple carcinomas. In this case, synaptophysin immunohistochemistry proved to be extremely helpful for the histological diagnosis.

7/122. Combined partial sternectomy for thymic carcinoma.

    For thymic carcinoma, a 67-year-old Japanese female underwent combined chemotherapy, to which a partial response was registered. Afterwards, total thymectomy with combined en bloc resection of the biopsy site, the partial sternum and the partial mediastinal components were completed with a clear margin. Skeletal reconstruction was performed using polypropylene mesh. Paradoxical movement of the chest wall was tolerable. Functional and cosmetic appearance of the chest wall following the reconstruction was satisfactory. Thirty-nine months had passed since the onset of the chemotherapy, when local recurrence was detected in the approximated major pectoral muscle. The patient underwent salvage radiation therapy that resulted in a complete response, and is currently alive and disease free at 46 months after the onset of the combined chemotherapy.

8/122. Non-Hodgkin's tumor and Pancoast's syndrome.

    A 60-year old man presented with Horner's syndrome, and acute right hand and lower extremity weakness. Chest X-ray and MRI revealed a right apical lung tumor (presumed to be a primary lung cancer), with brachial plexus infiltration and spinal cord compression. Emergent radiotherapy was initiated for spinal cord compression and a biopsy was obtained 24 h later. A careful review of pathology demonstrated a non-Hodgkin's lymphoma. The patient subsequently received chemotherapy, and is now in remission. This case illustrates the importance of a tissue diagnosis before initiating therapy for a Pancoast's tumor.

9/122. Extensive F-18 FDG uptake in metastatic spindle cell carcinoma of the lung.

    A 77-year-old man with stage IIB squamous cell carcinoma of the lung underwent right upper lobectomy. One month later he was examined for right chest pain, dyspnea, cough, and weakness. A roentgenogram showed nondiagnostic diffuse opacification of his right lung cavity. An F-18 FDG positron emission tomographic (PET) study revealed extensive uptake in the right pleural area, left adrenal gland, right axilla, and soft tissues consistent with extensive local recurrence and metastatic disease. biopsy of a right chest soft tissue lesion showed spindle cell carcinoma, a rare variant of squamous cell carcinoma.

10/122. [prenatal diagnosis of a brain tumour--an example of diagnostic and therapeutical algorithm]

    The authors describe their cooperation in the diagnosis and treatment of a newborn with malignant brain tumour (rare case of carcinoma of the choroid plexus) recognised by means of prenatal sonography and magnetic resonance. The case history is an example of modern algorithm of diagnostic and therapeutic procedures in perinatal medicine and the necessary multicentre collaboration.
