1/4. Primary aneurysm of the basilic vein.

    This is a report of a 71-year-old woman who presented with a superficial soft tissue mass in the antecubital fossa. Excision was undertaken under local anaesthesia. The lesion proved to be a primary aneurysm of the basilic vein. Reviewing the literature we found only a few previous reports of primary venous aneurysms in the upper extremities.

2/4. Repair of internal carotid aneurysm under local anaesthesia. Case report.

    Aneurysms of the extracranial carotid arteries are unusual events but cause death or a cerebrovascular accident in the majority of cases (50-70%). This report describes a true internal carotid artery aneurysm that developed within six weeks. The aneurysm was resected and the artery repaired by the use of a graft from the internal jugular vein. Carotid cross-clamping time was 93 minutes. The operation was performed under local anaesthesia, with continuous clinical monitoring of the ipsilateral cerebral hemisphere function.

3/4. Infected false aneurysm after puncture of an aneurysm of the deep femoral artery.

    Pseudoaneurysm of the femoral artery is a well known complication after diagnostic and therapeutic catheterisation. We report a case of voluminous postcatheterisation false aneurysm, which developed on a pre-existing deep femoral artery aneurysm. infection by staphylococcus aureus was demonstrated. After surgical exploration, rupture and external haemorrhage occurred before reconstructive surgery was possible. Due to the septic conditions only a lateral aneurysmorraphy was performed. Postoperative angiogram showed an acceptable result. An aneurysm of the deep femoral artery was also demonstrated on the contralateral side. This was scheduled for elective surgery. In conclusion, this complication could have been avoided by accurate puncture technique. Surgical exploration under local anaesthesia is ill-advised. Especially in patients with advanced vascular disease, non-invasive studies of the puncture site is recommended.

4/4. aneurysm of the right gastroepiploic artery: a case report of laparoscopic resection.

    A traumatic pseudoaneurysm of the right gastroepiploic artery in a 55-year-old woman was resected using laparoscopy. The durations of anaesthesia and operation were 260 and 220 min respectively. blood loss was minimal. The perioperative period was uneventful; the patient needed no analgesic treatment and ate her first meal on the morning of the day after surgery. She was discharged 5 days later and has progressed well for 5 months since surgery without any abnormal findings.
